Sandwich Note

A sandwich note is a subtype of progress note that reports on the status of a sandwich that a patient is eating, about to eat, or has just completed eating. In the event of a chart being reviewed in court before lawyers and a judge, the sandwich note is considered the most important document in the medical record. Not charting a sandwich note is punishable by 1-year suspension.


How to Write a Sandwich Note


  1. Always date and time the note
  2. Name of sandwich
  3. Type of bread and number of slices
  4. List any meats
  5. List any cheese
  6. List any toppings, such as lettuce, tomato, or avocadoes
  7. List any condiments
  8. Hunger pain score before sandwich (on scale from zero to infinity)
  9. Time of first bite
  10. Time of last bite
  11. Hunger pain score after sandwich (on scale from zero to infinity)
  12. Percentage of sandwich consumed
  13. Code status
