An exclamation point is one of the most underutilized forms of punctuation in medical documentation. In combination with the question mark, the exclamation point can help emphasis certain parts in the medical record.

Examples of Ways to Use the Exclamation Point in Medical Documentation

"GI bleed!!!!!!!!!!!!!!!!"

"The 101-year-old patient is full code?!?!"

"Nutrition has been consulted!!"

"Patient states his pain is controlled with Tylenol!"
