Gallops may represent pathology in older patients.[1] When you hear an S3 or S4 gallop, step back and make sure you’re not anywhere near a racetrack like Preakness or Belmont. As the old adage goes, Whenever you hear hoofbeats, always think horses. How embarrassing would it be to hear a murmur when it turns out you were just examining a thoroughbred?
