A complication of EHR, note bloat refers to electronic documentation the frequent use of copy-and-paste or cut-and-paste to the point that the note itself becomes distended with fluid or gas.[1][2][3] Medical documentation notes are bad enough as they are, let alone when they begin farting or sharting.[4]
