History
The first stone & chisel medical record was implemented at Cedar-Sinai Medical Center in Los Angeles, CA in an attempt to make their traffic appear relatively efficient in comparison. It did not take long for health care professionals to describe the stone & chisel medical record as "ridiculous" and a "pain in the a**," which is exactly what hospital administrators at Cedar-Sinai Medical Center were going for.
Most Common Causes of Death
The most common cause of mortality due to the stone & chisel medical record is being crushed to death. As of January 28, 2017, the death total is currently at three. Carpel tunnel syndrome and adult failure to chisel are other common maladies.
