Ummm, no it's not! If it was, it'd be the only subjective vital sign, which defeats the purpose of vital signs.[1] A respiratory rate that is always charted at 16 is still a more useful vital sign than pain as the 5th vital sign. Even the Press Ganey waveform is a more useful vital sign![2]
