As much as you can get away with without forcing dialysis.

Indications
Anything. If there's a fever, give it. If there isn't a fever, give it. But when you give it, always pair it with Zosyn. If Vanc is the King, then Zosyn is the Queen!
Adverse Reactions
There are probably some. But it's vancomycin!!!! You know you want to use it!
